Flintwood currently supports people with the Individual Accommodation Support Packages (IASP). IASPs are a new accommodation support option for people with a Disability that require ongoing support but do not wish to live in a full-time residential care service.
The funding is individualised, flexible and portable for people with medium to high support needs who want to live in a home of their own.
The packages of support provided by any Service Provider must not exceed the net cost of a place for a Service User with the same need profile in a 24/7 support model.
The IASP is not designed for people who need continuous or complex formal support (e.g. 24 hour or overnight staffing or nursing support).
What will the IASP provide?
The Service User, their family or Carer(s), and their Case Manager works with Flintwood to plan, manage and monitor the supports package, which may include assistance with:
- Personal care and daily living support
- Communication skills and positive behaviour supports
- Assistance with household activities including meal preparation, being a good neighbour, paying rent on time
- Maintaining social skills, and positive family and peer relationships
- Participating in leisure and recreation activities
- Getting around the community, including use of transport
- Day activities, training, employment and volunteer work
- Maintaining heath and well being
- Facilitating access to additional professional supports as required
- Budgeting and financial management
- Supporting co-ordination and / or intermediary service provision

[/vc_column_text][/vc_accordion_tab][vc_accordion_tab title=”Supported Living Fund” title_tag=”h3″][vc_column_text]Flintwood currently supports people under the Supported Living Fund (SLF). This form of Accommodation is designed to support adults with a disability to live in a home of their own, through a combination of funding and planned supports, enabling them to direct their own supports.
The funding is individualised, flexible and portable. It must be sustainable for the Service User and relies in part on informal ‘circle of support’ networks.
SLF packages are typically used to purchase a combination of traditional disability supports and mainstream services, complementing the Service User’s existing supports and networks.
The SLF can be used to:
- Develop and implement plans for moving into your own home
- Pay for support to live in your own home
- Attend life skills programs (e.g. cooking, work skills etc.)
- Increase circles of support and informal networks
- Purchase specialist accommodation and needs based equipment ( e.g. home modifications)
- Facilitate relationship building and social supports
The Service User may use the SLF as either an on-going or one off funding amount, and levels of assistance from the SLF may change over time to correspond with the Service User’s changing needs. The average amount of a SLF package is $50,000 per person per annum.
Who is eligible for the Supported Living Fund?
To be eligible for the SLF, you must be:
- An adult (18 – 64 years) with a disability as defined by the NSW Disability Services Act 1993;
- A resident of NSW; and
- Interested in living in a home of your own, with a mix of formal and informal supports that are adequate and sustainable.
How are applications made for the Supported Living Fund?
Individuals may self-refer or be nominated by their family / Carer, Case Manager or other Service Providers. If they wish to nominate themselves, they can go to the ADHC website for further information, including a SLF proposal form: www.adhc.nsw.gov.au/individuals/support/somewhere_to_live/slfregistrations[/vc_column_text][/vc_accordion_tab][vc_accordion_tab title=”Self Managed Model” title_tag=”h3″][vc_column_text]Flintwood currently supports people who are funded under the Community Participation or Life Choices program, in the Self Managed Model (SMM).
The aim of the SMM is that the Service User has flexibility to design their own customised program of support. The Service User can purchase and control their own support using Flintwood to manage their financial, legal and administrative requirements. This model is based on the fact that the Service User and family / Carer have the willingness and the capacity to direct their support and purchase relevant services.
The SMM has the following key features:
- The Service User chooses the people who work with them including paid support workers and other people they know such as family, friends and neighbours. These people may also work with the Service User on an unpaid basis, as part of their natural support network.
The Service User, Family / Carer, Flintwood develop a Personal Development Plan (PDP) and a budget. Flintwood will provide monthly statements to the Service User.
The Service User’s PDP has the flexibility to provide as many options as possible. This means activities may occur at times and in ways that best suit the Service Users needs. The plan must reflect a creative and flexible approach to developing support.
Flintwood does act as the legal employer of staff where required by the Service User. Flintwood takes responsibility for the payment of wages, insurances and all WHS requirements.
Flintwood acts as a point of contact and actively works to link Service Users and their families / Carers together to share their knowledge and experiences.
How are applications made for the Self Managed Model?
- If you already have funding, talk to Flintwood re transferring the SMM.
- If you are a school leaver, talk to your transition teacher about which program you are interested in
