Flintwood Disability Services provides many opportunities for young adults after they complete their secondary schooling. We offer the following post school programs:

Community Participation Program

  • Centre Based with Community Access
  • Individual Community Based Options

The Community Participation (CP) program has two service types to provide flexibility and individually focused activities. Both the Centre Based with Community Access and Individual Community Based Options are person centred, designed for young people, genuinely inclusive, flexible, cost effective, culturally competent and respectful.

Activities we offer include: Swimming, Hydrotherapy, Carpentry, Gardening, Bowling, BBQ’s, Movies, Shopping Programs, Pub, Drama Therapy, Art Therapy, Massage, Music Therapy, Darling Harbour, Circular Quay, Going on the ferry, beach and many other community venues.

Each young person has an Individual Program Plan (IPP) which will outline their goals. Based on this, the young person chooses certain activities which are on offer which he/she may like to participate in. Most activities are conducted in small groups (e.g. 4-6 people) involving young people with similar goals and interests. Some time may be spent one-on-one with people with higher support needs as and when needed (e.g. Personal Care or Behavioural Support).

 

Post School Options Program

This program provides support and services which enable school leavers to make the passage from school to community living and adult life roles, with all the opportunities associated with promoting their greater independence, increased productivity and maximum integration into the community.

Activities under this program will:

  • further develop skills learnt at school
  • acquire and use new personal, social and daily living skills essential for community participation
  • encourage greater responsibility for making choices and decisions
  • maintain social networks such as family and friends
  • increase access to and participation in day-to-day activities in local communities

Eligibility
All applicants must be PSO approved and funded by the Department of Ageing, Disability and Home Care to be eligible for this program. Flintwood will accept transfers from other PSO Programs.

 

Transition to Work Program

The Transition to Work (TTW) Program is a two year program which aims to achieve employment. The program develops the skills and qualifications needed to secure and sustain employment.

The Program provides services for young people with a range of support needs, however, is primarily targeted to young people with moderate to high needs.

The Program focuses on developing the following skills:
Social Skills, Travel Training, Independent Living Skills, Preparing Resume’s, Interview Techniques, Appropriate Behaviour in the Workplace, Personal Hygiene, OH&S, Appropriate Dress in the Workplace and Money Management etc.

Each young person in the Transition to Work Program has a Transition Plan that is person centred.
This means that Transition Plans are based on employment goals, current skills, experience and related learning, development and the support needs of each young person. This plan outlines the type of supports and activities to be provided and how these will meet the individual goals and needs of the young person. Transition Plans are reviewed every six months to ensure Flintwood is meeting the needs of the young person.

Transition to Work is purely a Vocational program.

 

Day Services

Day Services include support in living skills, peer support, recreation, training, and working towards independence. This is a block funded type of program and tends to be for adults who are not eligible for Community Participation, Transition to Work or Post School Options funding.

Flintwood Day Services are located at:
Baulkham Hills, Brookvale, Harris Park, Kingswood, Seven Hills, Pennant Hills, Penrith and Westmead.

Service Hours
The service is open between 8.00 am to 4.00 pm Monday to Friday. However, hours and number of days of attendance can vary according to individual needs, funding allocations and are worked out in consultation with the Service User, family and carers.

Day Services close for three weeks over the Christmas and New Year period and one week mid year, generally in July.

Transport

Transport to and from the service is the responsibility of the individual, family or carers to arrange. Funding does not cover transport service, however we may be able to assist you with contacts if needed. During the day all Service Users will travel to their activity by Public Transport or the service vehicles, driven by members of staff.

Fees

Service Fees:
Service Users will pay an attendance fee and will vary depending on the program type

Activity Fees:
All Service Users participating in the program will be required to pay for activities undertaken (movies, swimming, bowling, lunch etc).

User Pay:       
Individuals can buy services privately, please request more information from Flintwood administration.

 *Post School Programs and Activity Fees are billed monthly.
